J Pediatr Surg, 2010)\n<ul class=\"wp-block-list\">\n<li>30% of ovarian torsions are in adolescents (Ashwal et al. Obstet Gynecol, 2015)<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Can also see pain with Mullerian anomalies and onset of menses if non-communicating (remnant uterine horn, high vaginal septum)<\/li>\n<\/ul>\n\n\n\n<p><em>Premenopause<\/em><em><\/em><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Torsion is suspected whenever the mass is &gt;6 cm<\/li>\n\n\n\n<li>Pregnancy related adnexal masses can present with localized or diffuse abdominal pain\n<ul class=\"wp-block-list\">\n<li>Ectopic pregnancy (see \u2018Related Topics\u2019 below)<\/li>\n\n\n\n<li>Corpus luteum cyst: Larger in early pregnancy, will recede | Often diagnosed in setting of bleeding and pain in early pregnancy, are not a source of pain<\/li>\n\n\n\n<li>Theca lutein cysts: Seen primarily in women with ovarian hyperstimulation, IVF cycles, but can be seen in multiple gestation or molar pregnancy | Can be associated with diffuse abdominal pain and significant bloating<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Other gyn diagnoses on the differential in this age group include\n<ul class=\"wp-block-list\">\n<li>Hemorrhagic cyst<\/li>\n\n\n\n<li>Endometrioma<\/li>\n\n\n\n<li>Mullerian anomaly<\/li>\n\n\n\n<li>Necrosing pedunculated leiomyoma<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Less likely to cause pain, but could be incidental finding\n<ul class=\"wp-block-list\">\n<li>Functional cyst<\/li>\n\n\n\n<li>Dermoid cyst<\/li>\n\n\n\n<li>Cystadenoma<\/li>\n\n\n\n<li>Leiomyoma<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Infectious\n<ul class=\"wp-block-list\">\n<li>TOA (see \u2018Related Topics\u2019 below)<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Bowel disorder\n<ul class=\"wp-block-list\">\n<li>Appendicitis<\/li>\n\n\n\n<li>Diverticulitis<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<p><em>Postmenopause <\/em><em><\/em><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>With an ovarian mass, cancer must be considered and ruled out<\/li>\n\n\n\n<li>Unless urgent surgical abdomen, this is best referred to GYN oncology\n<ul class=\"wp-block-list\">\n<li>According to SEER data the average age at diagnosis of ovarian cancer is 63, and 69.4% of women are &gt;55 years at diagnosis (ACOG PB 174)<\/li>\n\n\n\n<li>Features of concern: size &gt;10 cm | Complexity | Solid components | Free fluid\/ascites | Flow on color doppler | Symptoms (bloating, abdominal distention) | Anorexia | Bowel obstruction)<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Torsion is less likely but a possibility in this population<\/li>\n\n\n\n<li>Infection\n<ul class=\"wp-block-list\">\n<li>TOA possible (see \u2018Related Topics\u2019 below)<\/li>\n\n\n\n<li>Diverticulosis | diverticulitis<\/li>\n\n\n\n<li>Appendicitis<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Bowel mass | Colon cancer<\/li>\n<\/ul>\n\n\n\n<p><strong>Note: <\/strong>Patient Age is Helpful in Determining Likelihood of Malignancy | While diagnosis would require surgical and pathologic evaluation, counseling of a patient in the ER can help manage expectations<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"work-up\">Work-Up<\/h3>\n\n\n\n<p><em>History<\/em><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><em>A complete menstrual history<\/em><\/li>\n\n\n\n<li><em>History of pelvic pain<\/em><\/li>\n\n\n\n<li><em>Sexual history<\/em><\/li>\n\n\n\n<li><em>Recent physical activity, including riding on rollercoasters<\/em><\/li>\n\n\n\n<li><em>History of pelvic masses or disorders such as endometriosis<\/em><\/li>\n<\/ul>\n\n\n\n<p><em>Physical exam<\/em><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><em>Vital signs for signs of volume depletion or acute bleeding, sepsis<\/em><\/li>\n\n\n\n<li><em>Abdominal exam including bowel sounds, rebound, guarding<\/em><\/li>\n\n\n\n<li><em>Pelvic exam for bleeding, purulence, cervical motion tenderness, palpable mass,<\/em><\/li>\n<\/ul>\n\n\n\n<p><em>Imaging<\/em><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Ultrasound (preferably transvaginal) is the preferred imaging modality for gynecologic related adnexal mass<\/li>\n\n\n\n<li>Other modalities have not been shown to be superior for diagnosis, though MRI can help distinguish between ovarian and other etiologies of pelvic masses (AIUM practice parameter)<\/li>\n<\/ul>\n\n\n\n<p><em>Laboratory testing<\/em><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>CBC for WBC and Hgb\/ Hct<\/li>\n\n\n\n<li>Pregnancy test<\/li>\n\n\n\n<li>Tumor markers are not immediately helpful, but may be sent in anticipation of need for oncology workup and include (AFP, CA-125, b-hCG, CEA, CA 19-9, Inhibin and Lactate dehydrogenase)<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"management\">Management<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Minimally invasive surgical options are preferred for management of adnexal masses\n<ul class=\"wp-block-list\">\n<li>Laparoscopic management is first line<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>If torsed ovary (or fallopian tube) found at laparoscopy it should be untorsed\/untwisted\n<ul class=\"wp-block-list\">\n<li>If a large cyst is present it can either be removed or I&amp;D<\/li>\n\n\n\n<li>Oopherpexy is not standard of care<\/li>\n\n\n\n<li>Many studies show that the appearance of black or blue ovary is not indicative of necrosis, and while 72 hour window for recovery of function has been proposed, there is evidence the ovary can recover after longer torsion time (Cohen et al. J Am Assoc Gynecol Laparosc, 2001)<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Fertility preservation and maximal preservation of ovarian tissue is paramount for premenopausal woman or adolescent<\/li>\n\n\n\n<li>Endometrioma resection is associated with decreased ovarian reserve\n<ul class=\"wp-block-list\">\n<li>Patients should be counseled regarding limited evidence to support surgical excision<\/li>\n\n\n\n<li>Reserve for suspicious features, progressive symptoms and large cysts<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Cyst size\n<ul class=\"wp-block-list\">\n<li>Simple cysts &lt;10 cm: Generally can be safely followed with low malignant potential even in post-menopausal women (Modesitt et al. Obstet Gynecol, 2003)<\/li>\n\n\n\n<li>&gt;6 cm: Torsion risk increases<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Cyst aspiration:\n<ul class=\"wp-block-list\">\n<li>&nbsp;Appropriate for TOA<\/li>\n\n\n\n<li>Should be avoided if any concern for malignancy<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"primary-sources\">PRIMARY SOURCES:<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"santos\">Santos et al. J Pediatr Adolesc Gynecol, 2015<\/h3>\n\n\n\n<p>This case series assessed 29 female children (mean age 10.9 yrs) who underwent laparoscopic detorsion<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The mean duration of pain was 77.5h (+\/- 78.8h)<\/li>\n\n\n\n<li>In 72.4% of cases the ovary was described as \u201cdusky or purple\u201d<\/li>\n\n\n\n<li>No patients required re-operation for removal of ovary<\/li>\n\n\n\n<li>No cases of ovarian vein thrombosis were detected<\/li>\n\n\n\n<li>At follow up U\/S (avg time to f\/u 8.1 months) 28\/29 showed follicles on the effected side<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"recommendations\">PROFESSIONAL RECOMMENDATIONS:<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">ACOG Committee Opinion 783<\/h3>\n\n\n\n<p>ACOG addresses torsion specifically in adolescents and emphasizes the following points in regards the management of suspected torsion<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>There are no clinical or imaging criteria sufficient to confirm the preoperative diagnosis of adnexal torsion<\/p>\n\n\n\n<p>Doppler flow alone should not guide clinical decision making<\/p>\n\n\n\n<p>A surgeon should not remove a torsed ovary unless oophorectomy is unavoidable, such as when a severely necrotic ovary falls apart&nbsp;<\/p>\n<\/blockquote>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"references\">REFERENCES: <\/h2>\n\n\n\n<p><a href=\"https:\/\/www.acog.org\/clinical\/clinical-guidance\/practice-bulletin\/articles\/2016\/11\/evaluation-and-management-of-adnexal-masses\" target=\"_blank\" rel=\"noreferrer noopener\">ACOG&nbsp; Practice Bulletin 174: Evaluation and Management of Adnexal Masses<\/a><\/p>\n\n\n\n<p><a href=\"https:\/\/www.acog.org\/clinical\/clinical-guidance\/committee-opinion\/articles\/2019\/08\/adnexal-torsion-in-adolescents\" target=\"_blank\" rel=\"noreferrer noopener\">ACOG Committee Opinion 783: Adnexal torsion in adolescents<\/a><\/p>\n\n\n\n<p><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/26244532\/\" target=\"_blank\" rel=\"noreferrer noopener\">Characteristics and management of ovarian torsion in premenarchal compared with postmenarchal patients (Ashwal et al.
