Bernardo, DO<\/strong>, an Assistant Professor of Medicine in the Division of Infectious Diseases at Montefiore Medical Center and the Albert Einstein College of Medicine<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">SUMMARY:<\/h2>\n\n\n\n<p>The Centers for Disease Control and Prevention (<span class=\"glossaryLink\" aria-describedby=\"tt\" data-cmtooltip=\"&lt;div class=glossaryItemTitle&gt;CDC&lt;\/div&gt;&lt;div class=glossaryItemBody&gt;Centers for Disease Control and Prevention&lt;\/div&gt;\" data-mobile-support=\"0\" data-gt-translate-attributes='[{\"attribute\":\"data-cmtooltip\", \"format\":\"html\"}]' tabindex=\"0\" role=\"link\">CDC<\/span>) makes recommendations for the use of PrEP as biomedical HIV prevention. Based on a <span class=\"glossaryLink\" aria-describedby=\"tt\" data-cmtooltip=\"&lt;div class=glossaryItemTitle&gt;Systematic review&lt;\/div&gt;&lt;div class=glossaryItemBody&gt;A systematic review refers to a a rigorous literature review, where the question, inclusion and exclusion criteria and search strategy plan are well defined prior to beginning the review. &#xA0;A major goal is to synthesize data from multiple papers, while reducing bias to the greatest extent possible.&amp;lt;br \/&amp;gt;Statistical analysis of the data is often performed using meta-analysis with various programs and tools available to determine effect sizes of interventions.&lt;\/div&gt;\" data-mobile-support=\"0\" data-gt-translate-attributes='[{\"attribute\":\"data-cmtooltip\", \"format\":\"html\"}]' tabindex=\"0\" role=\"link\">systematic review<\/span> of the literature, these evidence-based guidelines offer a practical approach to identifying patients who would be appropriate candidates for PrEP, ordering relevant tests and laboratory procedures, and how to conduct initial and follow-up visits for patients. Given the evolving nature of PrEP research, these recommendations will be revised and updated for clinical practice on a regular basis. This section will also review other supportive guidelines and statements from <span class=\"glossaryLink\" aria-describedby=\"tt\" data-cmtooltip=\"&lt;div class=glossaryItemTitle&gt;ACOG&lt;\/div&gt;&lt;div class=glossaryItemBody&gt;The American College of Obstetricians and Gynecologists&lt;\/div&gt;\" data-mobile-support=\"0\" data-gt-translate-attributes='[{\"attribute\":\"data-cmtooltip\", \"format\":\"html\"}]' tabindex=\"0\" role=\"link\">ACOG<\/span> and <span class=\"glossaryLink\" aria-describedby=\"tt\" data-cmtooltip=\"&lt;div class=glossaryItemTitle&gt;USPSTF&lt;\/div&gt;&lt;div class=glossaryItemBody&gt;U.S. Preventive Services Task Force&lt;\/div&gt;\" data-mobile-support=\"0\" data-gt-translate-attributes='[{\"attribute\":\"data-cmtooltip\", \"format\":\"html\"}]' tabindex=\"0\" role=\"link\">USPSTF<\/span><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">General Guidelines for PrEP&#xA0;<\/h3>\n\n\n\n<p><em>Safety and Efficacy<\/em><\/p>\n\n\n\n<p>PrEP, when used as indicated, has been determined to be safe and effective in the prevention of HIV transmission among diverse populations and should be a consideration to anyone who is sexually active, including:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Men who have sex with men and transgender women: 92% to 100% reduction in HIV transmission in clinical trials and real-world analysis<\/li>\n\n\n\n<li>Heterosexual women and men &#x2013; 63% to 93% reduction in HIV transmission<\/li>\n\n\n\n<li>Persons who Inject Drugs (PWID) &#x2013; 73.5% reduction in HIV transmission<\/li>\n\n\n\n<li>Adolescents who weigh over 35kg &#x2013; over&#xA0;95% reduction in HIV transmission<\/li>\n<\/ul>\n\n\n\n<p><em>Eligibility<\/em><\/p>\n\n\n\n<p>The United States Prevention Services Task Force (<span class=\"glossaryLink\" aria-describedby=\"tt\" data-cmtooltip=\"&lt;div class=glossaryItemTitle&gt;USPSTF&lt;\/div&gt;&lt;div class=glossaryItemBody&gt;U.S. Preventive Services Task Force&lt;\/div&gt;\" data-mobile-support=\"0\" data-gt-translate-attributes='[{\"attribute\":\"data-cmtooltip\", \"format\":\"html\"}]' tabindex=\"0\" role=\"link\">USPSTF<\/span>) recommends offering PrEP with effective antiretroviral therapy to persons at high risk of HIV acquisition<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><span class=\"glossaryLink\" aria-describedby=\"tt\" data-cmtooltip=\"&lt;div class=glossaryItemTitle&gt;FDA&lt;\/div&gt;&lt;div class=glossaryItemBody&gt;U.S. Food and Drug Administration&lt;\/div&gt;\" data-mobile-support=\"0\" data-gt-translate-attributes='[{\"attribute\":\"data-cmtooltip\", \"format\":\"html\"}]' tabindex=\"0\" role=\"link\">FDA<\/span> approved oral formulation is one pill taken once a day or intramuscular injection is once very eight weeks<\/li>\n\n\n\n<li>Either F\/TDF 200\/300mg (Truvada) or F\/TAF 200\/25 (Descovy) are currently approved for oral use and cabotegravir is approved for intramuscular use<\/li>\n\n\n\n<li>Insurance must provide medication coverage for preventive services with Grade A recommendation<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">CDC Guidelines for Screening Eligibility<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The&#xA0;<span class=\"glossaryLink\" aria-describedby=\"tt\" data-cmtooltip=\"&lt;div class=glossaryItemTitle&gt;CDC&lt;\/div&gt;&lt;div class=glossaryItemBody&gt;Centers for Disease Control and Prevention&lt;\/div&gt;\" data-mobile-support=\"0\" data-gt-translate-attributes='[{\"attribute\":\"data-cmtooltip\", \"format\":\"html\"}]' tabindex=\"0\" role=\"link\">CDC<\/span> recommends that providers should<\/li>\n<\/ul>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>Inform all sexually active adults and adolescents that PrEP can protect them from getting HIV<\/p>\n<\/blockquote>\n\n\n\n<p><strong>Note:<\/strong> We caution against seeing patients as levels of &#x201C;risk,&#x201D; particularly when it comes to sexual health | What it involves is taking a good sexual history and documenting what behavioral choices they make, then offering PrEP as an option for HIV prevention<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>PrEP is for people without HIV who could be exposed to HIV from sex or injection drug use <\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li><span class=\"glossaryLink\" aria-describedby=\"tt\" data-cmtooltip=\"&lt;div class=glossaryItemTitle&gt;CDC&lt;\/div&gt;&lt;div class=glossaryItemBody&gt;Centers for Disease Control and Prevention&lt;\/div&gt;\" data-mobile-support=\"0\" data-gt-translate-attributes='[{\"attribute\":\"data-cmtooltip\", \"format\":\"html\"}]' tabindex=\"0\" role=\"link\">CDC<\/span> recommends the following should be assessed for PrEP including\n<ul class=\"wp-block-list\">\n<li>Sexually active gay and bisexual men without HIV<\/li>\n\n\n\n<li>Sexually active heterosexual men and women without HIV<\/li>\n\n\n\n<li>Sexually active transgender persons without HIV<\/li>\n\n\n\n<li>Persons without HIV who inject drugs<\/li>\n\n\n\n<li>Persons who have been prescribed non-occupational post-exposure prophylaxis (PEP) and report behaviors that could expose them to HIV, or who have used multiple courses of PEP<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>For sexually active adults and adolescents\n<ul class=\"wp-block-list\">\n<li>Anal or vaginal sex in the past six months <strong>and<\/strong><\/li>\n\n\n\n<li>HIV-positive sexual partner (especially if partner has unknown or detectable viral load) <strong>or<\/strong><\/li>\n\n\n\n<li>Recent bacterial STI <strong>or<\/strong><\/li>\n\n\n\n<li>History of inconsistent or no condom use with sexual partner(s)<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>For persons who inject drugs\n<ul class=\"wp-block-list\">\n<li>HIV-positive injecting partner <strong>or<\/strong><\/li>\n\n\n\n<li>Shares drug preparation or injection equipment<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>All persons eligible for PrEP\n<ul class=\"wp-block-list\">\n<li>Documented negative HIV test result before prescribing PrEP <strong>and<\/strong><\/li>\n\n\n\n<li>No signs\/symptoms of acute HIV infection <strong>and<\/strong><\/li>\n\n\n\n<li>Normal renal function <strong>and<\/strong><\/li>\n\n\n\n<li>No contraindicated medications<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<p><strong>Note:<\/strong> While the <span class=\"glossaryLink\" aria-describedby=\"tt\" data-cmtooltip=\"&lt;div class=glossaryItemTitle&gt;CDC&lt;\/div&gt;&lt;div class=glossaryItemBody&gt;Centers for Disease Control and Prevention&lt;\/div&gt;\" data-mobile-support=\"0\" data-gt-translate-attributes='[{\"attribute\":\"data-cmtooltip\", \"format\":\"html\"}]' tabindex=\"0\" role=\"link\">CDC<\/span> compartmentalizes &#x201C;risk groups&#x201D; as listed above, clinicians should remember that sexual health does not fit neatly into static compartments | Sexuality and sexual behavior are fluid, and individuals may change their partners, behavior, and condom use over time and situations<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Other Considerations<\/h3>\n\n\n\n<p><em>Follow-up<\/em><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Patients taking PrEP can be followed closely by medical staff who are both clinical and non-clinical to assure proper usage and safety\n<ul class=\"wp-block-list\">\n<li>Current recommended follow-up every three months for oral PrEP or every two months for intramuscular PrEP<\/li>\n\n\n\n<li>Use of in-person and telemedicine visits<\/li>\n\n\n\n<li>Specific laboratory testing is indicated upon initial visit and subsequent follow-up<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<p><em>Women&#x2019;s Health<\/em><\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>The American College of Obstetricians and Gynecologists (<span class=\"glossaryLink\" aria-describedby=\"tt\" data-cmtooltip=\"&lt;div class=glossaryItemTitle&gt;ACOG&lt;\/div&gt;&lt;div class=glossaryItemBody&gt;The American College of Obstetricians and Gynecologists&lt;\/div&gt;\" data-mobile-support=\"0\" data-gt-translate-attributes='[{\"attribute\":\"data-cmtooltip\", \"format\":\"html\"}]' tabindex=\"0\" role=\"link\">ACOG<\/span>) recommends providers working with women at risk of HIV transmission be aware and up to date on this preventative health tool and encourage strong adherence as key to its effectiveness<\/p>\n<\/blockquote>\n\n\n\n<p><em>Future Directions<\/em><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Future delivery systems of PrEP are in various stages of clinical development and research and will likely become available as options in the future:\n<ul class=\"wp-block-list\">\n<li>Long acting injectables &#x2013; now <span class=\"glossaryLink\" aria-describedby=\"tt\" data-cmtooltip=\"&lt;div class=glossaryItemTitle&gt;FDA&lt;\/div&gt;&lt;div class=glossaryItemBody&gt;U.S. Food and Drug Administration&lt;\/div&gt;\" data-mobile-support=\"0\" data-gt-translate-attributes='[{\"attribute\":\"data-cmtooltip\", \"format\":\"html\"}]' tabindex=\"0\" role=\"link\">FDA<\/span> approved!<\/li>\n\n\n\n<li>Implants<\/li>\n\n\n\n<li>Long-acting oral
